THERAPEUTIC MENTOR WRAPAROUND SVCS.

TITLE: THERAPEUTIC MENTOR

JOB SUMMARY: Under case management/coordination system, provides home and community-based therapeutic mentoring services to youth who are at risk of out-of-home institutional care; maintains supportive relationship with youth and family; participates in crisis intervention and team planning.

RESPONSIBILITIES:
Under the supervision of a social worker, implements one on one goal-focused work between mentor and youth to help meet the individual needs of the youth. Provides home and community based monitoring and therapeutic mentoring activities for youth at risk of out-of-home institutional care. Activities focus on safe, educational, recreational, and cultural opportunities. Independent living skills, day-to-day self management and linkage of youth with community resources also may be part of the service. In addition, individual will provide support to child and family by providing immediate feedback to child to reinforce and support appropriate behavior. This individual will work in conjunction with teacher, mental health worker, or other professionals to support the child’s optimal academic growth and behavior management.

SHIFT: PER DIEM HOURS

QUALIFICATIONS:

  • B.A. in human services or related degree (i.e. education, psychology, sociology, etc). In lieu of a degree, seven years experience working in human services.

  • Proficient organization skills and ability to meet all paperwork time frames.

  • Ability to be flexible, responsible, attend to detail, and participate as a team member is essential.

  • Ability to secure respect, confidence, and cooperation of children, adolescents, and caregivers.

  • Ability to use the mentor-child relationship in ways that are therapeutic; goal-directed and in accordance with overall treatment goals.

  • Must be able to commit to working a minimum of 4 hours per week with a client for a minimum of one year.

  • Must have valid driver’s license, proof of auto insurance, and acceptable level of coverage.

  • Must be First Aid and CPR certified.

  • Physically able to perform the essential functions of the position, with or without reasonable accommodations, including ability to provide services in a variety of sites in the community, including client homes.
