FAMILY SUPPORT SPECIALIST I - MDFT |
PROGRAM: MULTIDIMENSIONAL FAMILY THERAPY (MDFT)
POSITION RESPONSIBILITIES: Provides intensive home based family substance abuse services to adolescents who are currently using substances or present with high risk behaviors. Services provided intensive case management, support services, advocacy, psycho-education and mental health services to adolescents and their families. MDFT operates often from a team approach and consists of a MDFT Clinician and MDFT Therapist Assistant both of whom participate in treatment meetings but also work independently with their identified treatment roles. Services provided are in collaboration with DCF, the court system, schools, and other community based providers. Team is responsible for attending weekly individual and group supervision, monthly state-side consultations and MDFT Booster Trainings. Responsibilities also include maintaining clinical documentation according to licensing, accreditation, Medicaid, and University of Miami requirements. Purposes of services are to stabilize the adolescent in the home and transition to traditional outpatient services, improve family functioning and communication, reduce further delinquency, help adolescent reduce or abstain from substance use and connect the adolescent with the community in a positive way. There is expected consultation and coordination care with Psychiatric providers if appropriate. Must maintain productivity targets determined by Service and remain current on all model training requirements.
QUALIFICATIONS:

BA or BSW with a minimum of 3 years experience in a comparable position in a human services agency may be substituted

Excellent oral and written skills and ability to communicate comfortably with children, parents, and school personnel

Organized and able to work independently with minimal supervision.

Flexible and able to multi-task.

Cultural competency to serve targeted populations.

Bilingual skills may be required.

Experience working with inner-city children.

Commitment to Results/Performance Based Accountability.

May be required to maintain valid driver’s license and ability to drive or otherwise obtain transportation to provide services throughout the service area.

Proficient with word processing and spreadsheet applications. Possess solid general computer skills, including ability to work in a Windows XP environment. Familiarity with specific applications used by the Village a plus.

Physically able to perform the essential functions of the position, with or without reasonable accommodations; able to function safely in a therapeutic situations including provision of services in clients homes or host settings; able to travel to various sites throughout greater Hartford.
